Too many forms ask, “Do you have any allergies?” and call it a day. But that question is too broad to be helpful. Most clients will simply say “no” because they’re thinking about food or seasonal allergies, not the adhesives, gel pads, or products you’re using.

The Fix:

“Do you have any known allergies or sensitivities to adhesives, latex, gel pads, or cosmetic products?”

  1. Include a Clear Cancellation & No-Show Policy

If your client intake form doesn’t require acknowledgment of your cancellation or no-show policy, you’re missing a crucial layer of protection. When issues arise, and they will, you’ll want written proof that the client agreed to your terms.

Include a simple, clearly worded section that requires a signature or checkbox:

“I understand that cancellations made within 24 hours of my appointment may incur a fee.”

Most lash artists want to grow their business, but very few are tracking where their clients actually come from. If you don’t know what marketing works, you’re just guessing.

“How did you hear about me? (Instagram / Google / Referral / Walk-in / Other: _____)”

This one question gives you insight into where to invest your time, energy, and possibly money. It also gives you the opportunity to thank loyal clients who send referrals your way.
